Case of the Year Report

Please fill out this form completely and attach a copy of the incident report and any supporting documents.  Provide a brief synopsis of the event in the space provided below.  If you need more space, attach a separate piece of paper. These will be voted on at Certification. These must be receive by the Secretary before certification (prior to October 6th, 2007 if you are not attending cert) to be eligible.  Only one entry per team allowed in each category.  Send the completed package to:
